Job summary

One Team One Trust is seeking a Nurse Practitioner to join the Sunderland Community Frailty Team. You will assess and co-ordinate care for older adults living at home or in care facilities, addressing frailty and complex needs within a MDT framework.

You will undertake CGAs, develop personalised care plans, and provide expert advice to enhance self-care and quality of life. You will work autonomously, with opportunities for innovation and professional development within the Trust.

Qualifications

• Registered Nurse with NMC registration and a nursing degree is essential.
• Mentorship or Practice Assessor training is desirable or demonstrated equivalent experience.
• Non Medical Prescriber or willingness to complete is preferred.

Responsibilities

• Coordinate delivery and development of the specialist service using evidence-based practice.

Job summary

We are looking for a highly motivated Nurse Practitioner to work within the Sunderland Community Frailty Team in Sunderland. The team consists of Band 7 and Band 6 Nurse Practitioners who are based in each of our six Community Integrated Teams. You will be responsible for out of hospital care assessment and co-ordination of care and services to people who live in their own home or nursing or residential care and are identified as having moderate to severe frailty, multiple co-morbidities complex needs or who are identified as high intensity service users. You will work as a member of the Multidisciplinary Team and will be required to attend and participate care home and community MDTs. You will be an autonomous practitioner.

You will be responsible for pro-actively case finding patients with moderate to severe frailty and undertaking CGAs Comprehensive assessment, personalised care planning, providing support and advice to enhance self-care and improve quality of life for older people and reducing level of frailty wherever possible.

You will work autonomously, assessing individual patient needs, initiating investigations, with support determining a differential diagnosis and initiating appropriate holistic, evidence-based treatment and care. You will refer patients who require medical intervention to the appropriate services.

Main duties of the job

As a Junior Nurse Practitioner in the Community Frailty Team you will be an experienced Registered Nurse, who preferably has experience at Band 6 level, you will have a great working knowledge of care of older people and patients with frailty and complex health needs and you will be a confident autonomous professional. You will be adynamic, approachable and self-motivated individual, you will be responsible for ensuring high standards are maintained and you will have a desire for continual improvement.

You will have:

• A full UK driving licence and access to a car
• Registered Nurse with NMC
• A nursing or related degree
• Advanced clinical assessment skills accreditation or willingness to work towards
• Non-medical prescribing qualification or willing to work towards extended prescribing skills
• Advanced decision-making skills

• To coordinate the delivery and development of the specialist service through the use of effective evidence based practice in line with agreed national standards for patient safety and monitoring.
