Job summary

Qualifications

• Experience as a community nurse or IV therapy delivery is advantageous.
• Strong clinical governance and patient safety focus.
• Able to perform venepuncture and cannulation when required.
• Excellent documentation and communication with multidisciplinary teams.

Responsibilities

• Deliver safe and effective IV therapy to patients in their own homes.
• Administer IV medications, fluids and therapies per prescribed plans.
• Undertake venepuncture and cannulation where required.
• Access and manage PICC lines, midlines and other vascular access devices where competent.
• Monitor patients before, during and after IV therapy; escalate changes in condition.
• Complete accurate clinical documentation following each patient visit; maintain infection prevention procedures.
• Provide patients and families with information and support; liaise with clinicians, pharmacies and hospitals.
• Work independently while maintaining high standards of clinical governance and patient safety.
• Travel between patient visits within the allocated community area.

• Recognise and appropriately elevate any changes in a patient's condition.
• Complete accurate clinical documentation following each patient visit.
• Follow infection prevention and control procedures.
• Provide patients and their families with appropriate information and support.
• Liaise with clinicians, pharmacies, hospitals and other healthcare professionals as required.
• Work independently while maintaining high standards of clinical governance and patient safety.
• Travel between patient visits within the allocated community area
