Norfolk and Suffolk NHS Foundation Trust (NSFT) is seeking a Band 5 Nurse for the Intensive Older Persons Service covering Great Yarmouth and Waveney locality in Lowestoft. The role involves assessment, planning, and delivery of care for older adults, with a focus on intensive community support and rapid response to referrals.

Full-time, permanent, with flexible working. Requirements include Nursing Degree or equivalent and mentorship or willingness to train; UK driving licence is essential.

Formación

• Excellent communication and organisational skills.
• Valid UK Driving licence is required for this post.
• Mentorship qualification or willingness to undertake training.
• Experience with CPA implementation or equivalent.

Responsabilidades

• Assess, plan, review and deliver care in collaboration with service users and carers, at home or in care homes.
• Assess risk and develop management and safety plans with service users and carers.
• Plan safe discharge and coordinate signposting and onward referrals.
• Conduct physical health checks and incorporate health promotion into practice.
• Maintain clear and timely documentation and liaise with carers, families and MDT.
