Qualifications

Responsibilities

  • Provide community outreach to each individual as part of the multi-disciplinary team.
  • Offer side-by-side interventions that focus on problem-solving, skill training, modeling, optimism, and encouragement.
  • Collaborate with the team and each individual to develop a person-centered treatment plan.
  • Support individuals in becoming fully integrated into their communities by helping them build natural supports.
  • Empower individuals to take control of their lives, focusing on their treatment, leisure activities, employment, and personal recovery.
  • Provide on-call coverage, as part of a rotation, during evenings and weekends.
