Job summary

Wye Valley NHS Trust is seeking an Integrated Discharge Team Nurse to work across County Hospital and community hospitals. The role requires a registered nurse (First/Second level) with strong problem-solving skills and a collaborative approach to care.

Qualifications

• Registered nurse with current NMC registration.
• Experience in discharge planning or care coordination is desirable.

Job description

The Discharge Nurse works within the Integrated Discharge Team working across the County Hospital (acute wards) and Community Hospitals (Bromyard, Leominster and Ross).

Main duties of the job

As a Integrated Discharge Team Nurse you will work in a collaborative way with Social Care colleagues to ensure the safe and timely discharges of patients that require support on discharge. Ensuring that the team ethos is adhered to by all who are involved in the discharge process at all times, 'Why not Home, why not today'. This will involve holding a caseload of patients with Nursing needs that require support on discharge.
