Job summary

Job description

Provide comprehensive, person-centred GP care to a defined cohort of housebound and care-home patients. Hold a weighted lost of approximately 130 patients per session, with appropriate prioritisation according to clinical need and complexity. Undertake proactive reviews of patients with frailty, multimorbidity and complex health needs. Provide face-to-face and remote clinical consultations as clinically appropriate.
