Job summary

Sirona care & health CIC is seeking an experienced Band 6 Case Manager to join the Integrated Network Team in the South Gloucestershire area. The role focuses on holistic assessments, care planning and coordinating support for individuals with complex health and social care needs.

You will work with GPs, social care, acute hospitals and care settings to ensure safe hospital discharge and seamless community care, contributing to service development and team leadership.

Qualifications

• Degree level qualification in Nursing, Occupational Therapy, Physiotherapy or Social Care.
• HCPC/NMC/Social Care England registration.
• Evidence of CPD.
• Relevant community experience.

Responsibilities

• Undertake holistic assessments for people with complex needs.
• Coordinate and manage a caseload after hospital discharge.
• Develop and review person-centered care and support plans.
• Support safe discharge planning using a Home First approach.
• Collaborate with health, social care and voluntary sector partners.
• Facilitate access to services, equipment and community resources.
• Provide advice on safeguarding, medication and long-term care.
