You are invited to join Suffolk County Council and North East Essex NHS Trust on an exciting Care Coordinator role in the Woodbridge Integrated Neighbourhood Team, helping people live well and live independently at home.

Job Title: Care Coordinator

Location: Woodbridge, Suffolk

Salary: £28,598 per annum (pro rata for part-time) – Local Government Terms and Conditions

Hours: 37 hours per week, flexible working options available

Contract: Permanent – Hybrid

  • Work collaboratively with professionals across health, social care, and voluntary sectors.
  • Navigate systems and services to ensure joined‑up, responsive care.
  • Support people through transitions in care, ensuring continuity and stability.
  • Promote preventative approaches to reduce hospital admissions and improve wellbeing.
  • Identify gaps in service provision and contribute ideas to improve delivery.
  • Contribute to team learning and development by sharing insights and best practices.
What You Need

  • Experience in health, social care, or community settings is helpful – transferable skills from other sectors welcome.
