• Ability to create personalized rehabilitation plans.
• Strong communication skills with patients and MDT.

Responsibilities

• Assessing patients and creating personalised rehab plans.
• Supporting or running groups such as falls prevention.
• Working closely with patients, families and the wider MDT.
• Identifying service developments and contributing to evaluations.
