Job summary

The Inherited Cardiovascular Condition service (ICC) at UHNM is recruiting a Clinical Nurse Specialist to deliver specialist nursing care for people with inherited cardiac conditions. You will build relationships with families, document detailed histories, and coordinate care across the multidisciplinary team, including liaising with consultants and other specialists.

You will communicate sensitive information with empathy and ensure continuity from diagnosis through to discharge.

Qualifications

• Ability to elicit and document detailed family history accurately.
• Experience communicating sensitive information to patients and families.
• Collaborate within a multidisciplinary team to deliver specialist nursing care.

Responsibilities

• Deliver specialist nursing service for inherited cardiac conditions.
• Coordinate care for families affected by ICC conditions.
• Liaise with consultants/registrars on treatment plans and referrals.
• Order and review diagnostic tests within parameters.
• Maintain up-to-date patient records and ensure continuity of care.
• Provide education and support to patients and families.
