We are seeking an Independent Prescriber to join our dedicatedteam. The successful candidate will play a key role in supporting ourclinicians, improving patient access to timely medication, and enhancing theoverall efficiency of our service,committed to delivering high-quality patient care.
You should be able to workindependently as well as part of a team, with excellent communication andorganisational skills. We would expect you to maintain up-to-date knowledge andskills through regular training and continuing professional development (CPD),ensuring that your practice remains safe, effective, and aligned with currentclinical standards.
This is a vital role in ensuring patientsreceive timely, safe, and effective care. You will be the first point ofclinical contact for patients, assessing their needs, prioritising care, anddirecting them to the most appropriate service.

Provide face-to-face triage for patients presenting with awide range of conditions.
Assess urgency and clinical risk, ensuring patients aredirected to the right care pathway.
Deliver safe, evidence-based advice and support topatients and carers.
Work collaboratively with GPs, Nurse Manager and other healthcare professionals.
Maintain accurate clinical records in line with NHSstandards.
Contribute to service development and quality improvementinitiatives.

Make appropriate referrals to primary, secondary and social care services, following practice (and/or locally agreed) pathways and protocols.
Deliver safe, evidence-based advice and support topatients and carers.
Work collaboratively with GPs, advanced nurse practitionerand other healthcare professionals.
Promote self-care and educate service users to allow them to be empowered to make informed choices about treatment
Maintain accurate clinical records in line with NHSstandards.
Contribute to service development and quality improvementinitiatives.
Cleaning and maintaining clinical equipment, reporting faults promptly
Assisting with the collection and collation of data on needs related to health and well-being and support QOF requirements and data completion
Actively participate in seasonal and special projects as requested e.g. flu campaign
Ensure adherence to confidentiality, data protection, and information governance requirements
Work safely at all times in accordance with legislative requirements and Practice Policy and Procedures
