Dryland Surgery in Kettering is seeking a clinical pharmacist to join the multidisciplinary team. The post focuses on patient-facing clinical assessment, structured medication reviews, and long-term condition clinics as part of integrated care in a general practice.

As an independent prescriber (or trainee), you will cover care home and housebound visits, telephone triage, and out-of-hours work, with involvement in medicines safety and cost-saving initiatives. GPhC registration is required.

Qualifikationen

• Mmasters degree in pharmacy (MPharm) or equivalent.

Aufgaben

• Hold structured face-to-face medication reviews and long-term condition clinics for multimorbidity.
• Manage own caseload and oversee team caseloads, including care home residents.
• Manage repeat prescribing reauthorisation and review dates.
• Reconcile medicines on hospital discharge with follow-up tests.
• Carry out care home medication reviews and liaise with staff.
• Perform domiciliary medication reviews and attend multidisciplinary case conferences.
• Identify high-risk patient cohorts using risk stratification tools.
• Telephone triage and refer patients to appropriate clinicians.
• Provide out-of-hours, on-call and extended-service consultations.
• Lead medicines safety, quality improvement and audits.
• Monitor prescribing against local formulary and support NICE-based services.
• Provide medicines information and training to colleagues.
