Greater Manchester Mental Health NHS Foundation Trust is seeking a Team Manager for the Inclusion Health Homeless Team in Salford. You will lead the city-wide team, oversee assertive outreach to those unable to access mainstream services, and carry a small caseload while coordinating with psychiatry, psychology and speech and language therapy on hand.

Qualifications

• Post-qualification experience of CPD.
• Significant post-qualification experience in a mental health setting relevant to the team.
• Experience or knowledge of working with co-occurring conditions is desirable.

Responsabilités

• Day-to-day operational management, leadership and development of the city-wide Inclusion Homeless Health Team.
• Deliver a safe, effective, high-quality service to a small caseload, in line with clinical pathways and operational procedures.
• Line manage the multidisciplinary team's dual diagnosis practitioners.
• Support engagement, longitudinal assessment and onward referral of homeless people who cannot access mainstream services.
• Take part in the Homeless Services Management Team, helping manage community homeless mental health and co-occurring conditions services.
• Work with Salford Local Authority Rough Sleeper Initiative housing team and the GMMH Rough Sleeper Drug & Alcohol service.
• Report against contractual key performance indicators.
