Sandwell Metropolitan Borough Council is seeking a motivated Shared Lives Reviewing Officer to join the Shared Lives Service within Adult Social Care in West Bromwich. You will assess, review and monitor individuals accessing Shared Lives services, working with carers to ensure safe, effective and person-centred placements.

You will develop and review person-centred support plans, undertake safeguarding and risk assessments, and collaborate with health professionals, families and partner

Qualifications

• Strong communication and interpersonal skills with the ability to build effective working relationships.
• Experience of working with vulnerable adults in a social care setting.

Responsabilités

• Assess, review and monitor individuals accessing Shared Lives services.
• Work with Shared Lives Carers to ensure placements are safe, effective and person-centred.
• Develop, implement and review person-centred support plans.
• Collaborate with families, health professionals and partner agencies to achieve positive outcomes.
• Undertake risk assessments and safeguarding activities to ensure welfare of vulnerable adults.
• Maintain accurate records and case management information.
• Contribute to the ongoing development and improvement of the Shared Lives Service.
