Job summary

Sanctuary Personnel is seeking an experienced Adult Social Worker to join the Early Help and Prevention Team in Medway. This ongoing role offers flexible full-time or part-time hours and a competitive hourly rate of £38.00, with the chance to tailor your schedule to fit your commitments.

You will undertake Care Act assessments, manage a complex caseload with strength-based interventions, collaborate with NHS and housing partners, and promote independence through preventative strategies and

Qualifications

• Degree in Social Work or equivalent recognised qualification.
• ASYE completion and evidence of continuing professional development.
• Registration with Social Work England and current practice eligibility.
• Experience within UK Adult Social Care and safeguarding practice.

Responsibilities

• Undertake Care Act assessments, reviews and safeguarding enquiries for adults with eligible needs.
• Manage a complex caseload with proportionate, strengths-based interventions.
• Review care packages and commissioned services for value and effectiveness.
• Promote independence via preventative interventions, resources and reablement.
• Collaborate with multidisciplinary teams including NHS, housing and voluntary sector.
