Qualifications

  • Registered with the General Pharmaceutical Council (GPhC).
  • Maintain GPhC registration and revalidation requirements.

Responsibilities

  • Own the onboarding and registration process for new patients.
  • Conduct medicines reconciliation consultations.
  • Liaise with GP practices to gather patient information.
  • Monitor vulnerable patients and repeat prescription processes.
  • Maintain accurate and confidential patient records.
  • Support the Clinical Lead and Practice Manager in governance.
