The Shrewsbury and Telford Hospital NHS Trust is seeking a consultant in general internal medicine for a role split between hospital-based care and the community virtual ward. • MRCP or equivalent.
• Full GMC registration and licence to practise.
• Entry on GMC Specialist Register via CCT or CESR (proposed CCT date within 6 months of interview).

Responsabilidades

• Diagnose and treat a wide range of clinical problems, from acutely ill younger patients to older patients with multiple complex co-morbidities.
• Look after patients outside a specific specialty or with complex multi-symptom problems, directing their treatment and coordinating specialist care.
• Provide continued care for this patient group in inpatient and outpatient settings.
• Carry out clinical activity as outlined in the job pack.
• Support a complex patient multidisciplinary team.
• Support links into community care, including virtual ward environments.
• Supervise higher specialty trainees, IMT stage 1 trainees and clinical fellows working towards CESR in general internal medicine.
• Take on formal education and clinical supervisor roles, with suitable training.
• Develop enhanced clinical skills, especially in assessing and managing acutely ill patients.
• Work effectively within a multi-professional team.
• Deliver clinical and quality targets agreed nationally and locally with clinical teams and commissioners.
• Follow patients throughout their clinical pathways.

A consultant post split between general internal medicine at The Shrewsbury and Telford Hospital NHS Trust and the community virtual ward, sitting within the General Medical Consultant team. The work centres on diagnosing and treating patients with complex, multiple co-morbidities who fall outside a single specialty, both as inpatients and in outpatient clinics.
