A prominent NHS organization in Salford is seeking an experienced social worker to join their specialist team. The successful candidate will provide social work support to patients, particularly those suffering from strokes and long-term neurological conditions. The role involves assessments for safe discharges and collaboration within a multi-disciplinary team. An opportunity has arisen for a social worker to permanently join the specialist team based at Salford Royal Hospital, providing specialist social work support to patients who have suffered a stroke and have other long‑term Neurological Conditions. We invite experienced qualified social work practitioners (at least level 2 to level 3 post‑qualified) to apply for this permanent full‑time post.
You will join an experienced and supportive Neurosciences Multi‑Disciplinary Team working across various wards and clinics. You will gain valuable social work experience by working with this client group whose needs can be complex and challenging but very rewarding. You will provide social work assessments for the safe discharge of Salford residents, set up services as required, attend MDT and professional meetings, and complete safeguarding investigations as required.
Ideally you will have some experience of working with this client group and of working in a hospital setting or similar.
