A community healthcare provider in Welwyn is seeking a Community B6 Nurse to join their Hospital at Home team. The successful candidate will work closely with integrated care teams to support patients with complex needs and manage long-term conditions while helping them stay in their homes. Candidates must be registered nurses with experience at the band 5 level and possess strong communication skills. We are excited to offer a Community B6 Nurse post to join our growing Hospital at Home team. The successful candidate will work closely and collaboratively with our award winning Integrated Care team, GP's and other providers to support a range of patients with complex needs to remain in their own home, avoiding hospital admission where possible following agreed pathways to manage acute illness and manage long term conditions.
