Job summary

Sheffield City Council’s Hospital First Contact Team is seeking a Social Care Practitioner to support hospital discharge planning and multidisciplinary working. You will conduct strengths-based assessments, provide information and advice, and collaborate with social workers, discharge nurses and partners to enable safe, timely discharges.

The role is hospital-based, working across acute sites, with involvement in Out of Hours rota and a focus on promoting independence for adults and their carers.

Qualifications

• Experience working with adults with care and support needs.
• Commitment to strengths-based and person-centred practice.
• Knowledge of the Care Act 2014 and safeguarding responsibilities.
• Ability to communicate effectively with a wide range of people and professionals.
• Organised, resilient and able to manage competing priorities.

Responsibilities

• Support timely and effective discharge planning within Sheffield Teaching Hospitals.
• Undertake strengths-based assessments and reviews of care needs.
• Provide information, advice and guidance to individuals, carers and professionals.
• Identify risks and support safeguarding processes where appropriate.
• Collaborate with health colleagues and partner agencies to achieve positive outcomes.
• Promote independence through community resources and technology-enabled care.
