An exciting opportunity has arisen for an experienced and motivated Band 6 Clinical Practitioner to join the West Essex Hospital at Home Service. Hospital at Home provides safe, effective and person-centred care to patients who would otherwise require admission to an acute hospital or who can be supported to leave hospital earlier. Care is delivered through a combination of face-to-face clinical assessment, treatment and remote monitoring, supported by a multidisciplinary team. The successful candidate will work autonomously within their professional scope of practice, managing patients with a range of acute and complex health needs in their own homes. You will undertake comprehensive assessments, develop individualised care plans, deliver clinical interventions and recognise and respond to deterioration. We are looking for a compassionate and forward-thinking practitioner with strong clinical decision-making, communication and organisational skills. Community or acute-care experience would be beneficial, although appropriate induction, training and ongoing professional development will be provided. The service operates seven days a week, and the postholder will be required to work flexibly across the service’s operating hours, including weekends and bank holidays.
