Job Description
  • Maintain confidentiality surrounding patients on the H@H caseload.
  • Ensure documentation is completed accurately and within agreed timescales using Hospital approved documentation, in line with NMC and Hospital guidelines.
  • Have knowledge of and adhere to Hospice and Hospital policy and professional standards; ensure team members do likewise.
  • Demonstrate a professional approach to work and act in accordance with the Hospital Code of Conduct and the NMC Code of Professional Conduct at all times.
  • Participate in staff appraisal, staff development and service training activities.
  • Meet Revalidation requirements; identify own learning needs and maintain professional development by keeping up to date with practice developments.
  • Undertake further training and academic qualifications as relevant to the role and service requirements.
  • Seek opportunities to develop the service.
  • Ensure that appropriate and timely data is complete to allow reports to be sent to commissioners.
  • Ensure that all leave, whatever the reason is, is managed appropriately.
  • Ensure all regulated activity is managed appropriately.
Person Specification

Desirable:

  • NMC Level 1 Registration.
  • Post registration qualification in palliative care.
  • Degree or equivalent qualification in a relevant discipline or teaching qualification.
  • Advanced Communication Skills Training.
  • Significant experience of working in a Palliative Care setting, either in the community or inpatient.
  • Experience working with members of a multi-disciplinary team.
  • Experience coordinating with external teams e.g. Continuing Health Care, Community Nursing.
  • Understanding and commitment to the values of our organisation.
