We are expanding our Hospice at Home service into Hammersmith Fulham and are seeking to recruit a Nurse Manager for the Hospice to lead on this exciting development.

Main duties of the job

The Hospice@Home Service is designed to deliver end of life and respite care for patients with specialist palliative care needs that wish to remain in their own home. Care is delivered in the patients home by Health Care Assistants.

The post holder will be responsible for the management of the St Johns Hospice @Home service which provides home care for patients living with in our catchment area of Westminster, Kensington, Chelsea, South Brent and Hammersmith and Fulham.

The post holder will be responsible for the overall management of the service. You will work with two clinical support nurses and a team of Health Care Assistants. You will be supported in your role by the Community Specialist Palliative Care team lead.

Job responsibilities

Tomaintain confidentiality surrounding patients on the H@H caseload.

Toensure documentation is completed accurately and within agreed timescales usingHospital approved documentation,in line with NMC and Hospital guidelines.

Tohave knowledge of and adhere to Hospiceand Hospital policy and professional standards. To ensure that team members dolikewise.

Todemonstrate a professional approach to work, and act in accordance with theHospital Code of Conduct and the NMC Code of Professional Conduct at all times.

Toparticipate in staff appraisal, staff development and in service trainingactivities.

Tomeet Revalidation requirements. To identify own learning needs and ensure ownprofessional development is maintained by keeping up to date with practicedevelopments.

Toundertake further training and academic qualifications as relevant to the roleand service requirements.

Seekopportunities to develop the service.

Ensurethat appropriate and timely data is complete to allow reports to be sent tocommissioners.

Toensure that all leave, whatever the reason is managed appropriately.

Ensureall regulated activity is managed appropriately

Person Specification
Job Criteria
  • Degree or level or equivalent qualification in relevant discipline teaching qualification
  • Advanced Communication Skills Training
  • Significant experience of working in a Palliative Care either in the community or an inpatient care setting
  • Experience of working with members of a multi-disciplinary team
  • Experience of co-ordinating with external teams eg Continuing Health Care, Community Nursing
  • Understanding and commitment to the values our organisation
