A healthcare provider in Bristol seeks an experienced Home Visiting Nurse to join their clinical team. The role involves managing care for house-bound patients, specifically focusing on Long Term Conditions. Responsibilities include conducting home visits, coordinating with healthcare professionals, and educating patients about their health. We are looking for an experienced Home Visiting Nurse to come and join our highly skilled, friendly clinical team.
Working underthe supervision of the Practices Community Nurse Practitioner (CNP) alongside3 other Community Nurse Practitioners. Strictly in accordance with specific practiceguidelines and protocols, the post holder will assist the practice clinicalteam in the provision and delivery of prescribed programmes of patient caremainly for house bound patients and specifically linked to long term conditionsmonitoring and management.
The postholder, who acting within their professional boundaries, will provide carespecifically but not limited to the practices elderly population and thoseassessed as being at high risk of emergency admissions to hospital. They will work collaboratively with themulti-disciplinary general practice team under the supervision of the PracticesCommunity Nurse Practitioner (CNP) to support the needs of practice patients.

Act as the co-ordinating person between the patient and the CNP/GP for this cohort.
Act as liaison with secondary care when patients are admitted and on planning discharge

Duties and responsibilities:
Homevisits specifically to patients with Long Term Conditions
Actas the co-ordinating person between the patient and the CNP/ GP for thiscohort.
Actas liaison with secondary care when patient's are admitted and on planningdischarge.
Actas the appropriate person that patients and carers contact in the practice todiscuss changes in the care plan or deterioration in condition.
Educatepatients and carers about the important indicators of any deterioration intheir chronic conditions so that appropriate action can be planned and takenbefore they reach the point of needing hospital admission. Utilising communityservices whenrequired.
Adhoc attendance at daily clinical meeting and monthly multidisciplinary meeting.
LongTerm Conditions management / annual reviews. When necessary supportingCNPs to refer onto Bristol CommunityHealth district nursing/long term conditions team.
Residentialhome visit e.g. Quarry House, The Elms and Worthies, Frenchay Park
DiabeticFoot Checks
Spirometry(desirable or willing to undertake training)
ECGrecording
Assistingin the assessment and surveillance of patients health and well-being withregard to their LTCs
Undertakingspecific clinical activities for named patients that have been delegated andtaught specifically in relation to that individual.
Helpingto raise awareness of health and well-being and how it can be promoted
Assistingwith the collection and collation of data on needs related to health andwell-being
Casecoordinator specifically linked to the Unplanned Admissions enhanced service.Working with clinicians and primary health care team and other agencies to supportthis service. This will include:
Patients will have been identified usinga risk stratification tool but this will need to be managed as people come ontoor drop off this cohort
Developing and reviewing care plans inaccordance and alongside the Practices Community Nurse Practitioners
Home visits
Working with admin and reception staffto coordinate caseload management effectively.
Act as the co-ordinating person betweenthe patient and the CNP/ GP for this cohort.
Undertaking hospital discharge reviewswithin 3 working days when directed by the CNP
Other may include and not limited to:
Chaperoningduties
Processingand management of laboratory samples requested by GPs/nurses
Sterilising,cleansing and maintenance of surgical equipment
Vaccine/coldchain storage, monitoring and recording
Surgicalequipment and vaccine re-stocking and stock rotation
Preparingand maintaining environments and equipment before, during and after patientcare interventions including assisting GPs during the performance of minoroperations

Health & Safety:
The post-holder will implement and lead on a full range ofpromotion and management their own and others health and safety and infectioncontrol as defined in the practice Health & Safety policy, the practiceHealth & Safety manual, and the practice Infection Control policy andpublished procedures. This will include (but will not be limited to):
Using personal security systems within the workplace accordingto practice guidelines
Awareness of national standards of infection control andcleanliness and regulatory / contractual / professional requirements, and goodpractice guidelines
Responsible for the correct and safe management of the specimensprocess including collection, labelling, handling, use of correct and cleancontainers, storage and transport arrangements
Management and maintenance of Personal Protective Equipment(PPE) for the practice including provision, ordering, availability and ongoingcorrect usage by staff
Responsible for hand hygiene across the practice
Ownership of infection control and clinically based patient careprotocols, and implementation of those protocols across the practice
Active observation of current working practices across thepractice in relation to infection control, cleanliness and related activities,ensuring that procedures are followed and weaknesses / training needs areidentified, escalating issues as appropriate
Identifying the risks involved in work activities andundertaking such activities in a way that manages those risks across clinicaland patient process
Making effective use of training to update knowledge and skills,and initiate and manage the training of others across the full range ofinfection control and patient processes
Monitoring practice facilities and equipment in relation toinfection control, ensuring that provision of hand cleansing facilities, wipesetc are sufficient to ensure a good clinical working environment. Lack offacilities to be escalated as appropriate.
Safe management of sharps procedures including training, use,storage and disposal
Using appropriate infection control procedures, maintaining workareas in a tidy, clean and sterile, and safe way, free from hazards. Initiationof remedial / corrective action where needed or escalation to responsiblemanagement
Actively identifying, reporting, and correction of health andsafety hazards and infection hazards immediately when recognised
Keeping own work areas and general / patient areas generallyclean, sterile, identifying issues and hazards / risks in relation to otherwork areas within the business, and assuming responsibility in the maintenanceof general standards of cleanliness across the business in consultation (whereappropriate) with other sector managers
Undertaking periodic infection control training (minimum twiceannually)
Routine management of own team / team areas, and maintenance ofwork space standards
Waste management including collection, handling, segregation,container management, storage and collection
Spillage control procedures, management and training
Decontamination control procedures, management and training, andequipment maintenance
