Pinnacle Home Care, Florida’s largest Medicare-certified home health provider, seeks a PRN Speech Language Pathologist (SLP) to join an award-winning team delivering care in patients' homes.

You will evaluate, assess, and treat speech, language, and hearing disorders, collaborate with physicians and interdisciplinary teams, and educate patients and families on therapy techniques while documenting progress and outcomes.

Qualifications

• Graduate of an accredited Speech Therapy program.
• Licensed Speech Therapist in Florida.
• Minimum of two years of clinical experience as a Speech Therapist.
• Current BLS Certification.
• Valid driver’s license and insured, reliable vehicle.
• Strong emotional resilience and physical stamina, including the ability to stand, stoop, and bend frequently.
• Excellent judgment, communication, and observational skills. Self-motivated with the ability to work independently.

Responsabilités

• Evaluate clients’ speech, language, and hearing abilities and recommend strategies.
• Conduct initial and ongoing assessments, including OASIS evaluations.
• Develop and revise treatment plans in consultation with physicians as necessary.
• Collaborate with interdisciplinary team members on care plans and provide speech pathology training to team members.
• Educate clients and families on speech therapy techniques and related challenges.
• Document all evaluation data, treatments, and client progress promptly.
• Supervise Home Health Aides every 14 days when Speech Therapy is the sole discipline involved.
• Communicate regularly with physicians, ensuring orders are appropriate and addressing changes in client condition.
• Participate in periodic revision of care plans and update physicians on client status.
