Frontpoint Health in Dallas–Fort Worth is seeking a Home Health Speech Language Pathologist (PRN) to provide in-home evaluation and treatment services for patients with communication and swallowing disorders.

You will develop individual treatment plans, work with patients with memory impairments, educate families, document progress, and coordinate with other healthcare professionals while complying with regulations.

Qualifications

• Master's degree in Speech Language Pathology.
• Current state license to practice as a Speech Language Pathologist.
• Minimum of 1 year of experience in home health.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong organizational and time management skills.

Responsabilités

• Conduct assessments to determine patients' speech and language abilities.
• Develop treatment plans to help patients improve their speech and language skills.
• Work with patients who have cognitive and memory impairments to develop strategies to improve their memory and cognitive function.
• Provide education to patients and their families on speech and language disorders and treatment options.
• Document patient progress and communicate with other healthcare professionals as needed.
• Adhere to all state and federal regulations and guidelines.
