Home from Hospital Zone Care Coordinator

• Location : Citywide Manchester (hospital sites across North / Central / South Manchester, community / home visits), with coordination hub at our Greater Manchester office
• Salary : £13.95 per hour (approx. £31,057.58 pa including on-call payments)
• Hours : 40 hours per week

Specialist Care Team is a values-driven social care provider. We deliver a citywide older people’s hospital discharge support that helps people get home safely and stay well for up to four weeks after discharge.

The service operates 7 days, removes non-clinical barriers (food, heat, safety), provides escorted transport, and connects people into community support to reduce readmissions. We work to clear flow pressures in hospitals and meet clear KPIs agreed with Manchester City Council and NHS partners.

The Zone Coordinator provides day-to-day leadership, coordination, and operational support across one of three zones (North, Central / East, South) within the Home from Hospital service. This vital community-based service supports Manchester residents aged 60+ to leave hospital safely, settle back home, and stay well for up to four weeks. To ensure the service is delivered effectively, you will also take part in a shared on-call and standby rota, which may include evenings and weekends.

Job Overview

(Note : In addition to these functions, employees are required to carry out such duties as may reasonably be required.)

• Provide day-to-day leadership and coordination for the Home from Hospital service within one of three Manchester zones (North, Central / East or South).
• Act as the operational link between hospital discharge teams, community partners, volunteers and frontline Support Workers.
• Ensure all new referrals are contacted and support initiated within 4 operational hours of receipt.
• Plan and oversee safe escorted transport home from hospital using company electric vehicles (EVs) or approved alternatives.
• Carry out or oversee home safety checks, settle-in visits and minor adaptations (e.g. key safes, grab rails) to facilitate safe discharge.
• Coordinate up to four weeks of post-discharge support, assigning visits, follow-up calls and goal reviews for each client.
• Supervise and support Support Workers and volunteers through regular communication, shadowing, and performance reviews.
• Liaise daily with ward staff and multidisciplinary teams (MDTs) to unblock barriers to discharge and maintain hospital flow.
• Maintain accurate, GDPR-compliant service records, reporting on KPIs such as 4-hour contact rates, satisfaction and income maximisation.
• Identify risks or safeguarding concerns promptly, following SCT and Manchester City Council procedures.
• Ensure patients have food, utilities and key safes at home and record all expenditures.
• Promote teamwork, wellbeing, equality and SCT values across your zone to deliver excellent person-centred outcomes.
