A community-focused care provider in Manchester is seeking a Home from Hospital Zone Care Coordinator to lead and coordinate the hospital discharge support services. You will manage operational links between hospital and community partners, ensuring timely support for older residents returning home from hospital. The ideal candidate should have strong leadership and coordination skills, along with the ability to manage time effectively and support a team. This role offers a salary of £13.95 per hour, with full-time hours and various worksite responsibilities.
Specialist Care Team is a values-driven social care provider. We deliver a citywide older people’s hospital discharge support that helps people get home safely and stay well for up to four weeks after discharge.
The service operates 7 days, removes non-clinical barriers (food, heat, safety), provides escorted transport, and connects people into community support to reduce readmissions. We work to clear flow pressures in hospitals and meet clear KPIs agreed with Manchester City Council and NHS partners.
The Zone Coordinator provides day-to-day leadership, coordination, and operational support across one of three zones (North, Central / East, South) within the Home from Hospital service. This vital community-based service supports Manchester residents aged 60+ to leave hospital safely, settle back home, and stay well for up to four weeks. To ensure the service is delivered effectively, you will also take part in a shared on-call and standby rota, which may include evenings and weekends.
