Job summary

East Sussex Healthcare NHS Trust is recruiting Integrated Support Workers to travel across local communities, delivering essential care in patients’ own homes and helping people maintain independence in familiar surroundings.

You will work as part of a multidisciplinary team, providing compassionate support, monitoring wellbeing, and coordinating with colleagues to ensure timely, respectful care within the Home First and Community services.

Qualifications

• Must have a full UK driving licence and access to a car.
• Experience delivering care in nursing or social care settings.
• Good standard of education including Maths and English GCSEs.

Responsibilities

• Travel across the local community to deliver care in patients' homes.
• Work independently within a team to support patients and maintain independence.
