Job summary

Mid Cheshire Hospitals NHS Foundation Trust is recruiting an experienced Therapist to support Home First within the IPOCH team. The post holder will work across the Emergency Department, admission avoidance pathways, inpatient wards and community settings to enable timely decisions and appropriate care settings.

You will lead in discharge planning, contribute to MDT meetings, and support education and supervision of colleagues while delivering high-quality, patient-centred care.

Qualifications

• HCPC registration required.
• Experience in discharge planning and Home First initiatives.
• Strong communication and teamwork in multidisciplinary settings.

Responsibilities

• Coordinate discharge planning across ED, admission avoidance pathways, inpatient wards and community settings.
• Support Home First principles and complex home visits with the MDT.
• Provide clinical and professional leadership, supervision, and education within IPOCH.
