Job Overview

Provide care for patients transferred from UCLH to the community prior to their estimated discharge date. The post holder will lead the team, take responsibility for case‑finding, coordinate the transfer team, and organise the transfer home.

This role is part of the UCLH@HOME service, a partnership between Whittington Health and UCLH, which helps patients leave hospital once medically optimised and safely managed in the community while remaining under the care of the consultant team.

Responsibilities
  • Provide direct care delivery within the patient’s own home.
  • Use remote‑monitoring technologies.
  • Lead the team, coordinate case‑finding, team coordination and transfer organisation.
  • Ensure safeguarding and child protection procedures are followed.
