A healthcare organization in the United Kingdom is seeking a dedicated professional to lead a team in providing patient care for those transitioning from a hospital to their home. The role involves coordinating community health services, ensuring safe patient transfers, and utilizing remote monitoring technology. Ideal candidates will have experience in emergency nursing, primary care, or acute medicine. This position offers an opportunity to enhance autonomous practice within a supportive framework.
Provide care for patients transferred from UCLH to the community prior to their estimated discharge date. The post holder will lead the team, take responsibility for case‑finding, coordinate the transfer team, and organise the transfer home.
This role is part of the UCLH@HOME service, a partnership between Whittington Health and UCLH, which helps patients leave hospital once medically optimised and safely managed in the community while remaining under the care of the consultant team.
