Responsibilities

  • Provide support and time to an allocated group of service users and promote their recovery.
  • Assist the multi‑disciplinary team to assess, plan, implement and evaluate individual care plans.
  • Be an active member of the multi‑disciplinary team (MDT).
  • Encourage and support clients to collaborate in their treatment programme and recovery.
  • Encourage self‑help, promoting the maintenance of skills and independence in clients.
  • Support health promotion and act in a consultative role to other community services regarding individual clients.
  • Undertake planned interventions and provide feedback outcomes to MDT.
  • Utilise highly developed interpersonal skills to facilitate communication.
  • Establish and maintain robust communication networks with clients, carers, and other health care workers.
  • Promote recovery and recovery‑oriented practice through active collaboration.
Job description
Overview

The post holder will work as part of the South Manchester Home Based Treatment Team, undertaking allocated duties within the team. They will work alongside mental health practitioners to support patients, families and carers in their own homes and to support patients who are on the community waiters and supported discharge pathway from the wards, ensuring the highest possible standard of care in accordance with policies and procedures.

Service users will have individualised care plans. The role focuses on the direct needs of service users and promotes social inclusion and active participation in an individualised recovery plan using agreed team values and skills.

Responsibilities
  • Provide support and time to an allocated group of service users and promote their recovery.
  • Assist the multi‑disciplinary team to assess, plan, implement and evaluate individual care plans.
  • Be an active member of the multi‑disciplinary team (MDT).
  • Encourage and support clients to collaborate in their treatment programme and recovery.
  • Encourage self‑help, promoting the maintenance of skills and independence in clients.
  • Support health promotion and act in a consultative role/advisor to other community services/agencies and personnel regarding individual clients and their mental health issues.
  • Undertake planned interventions and provide feedback outcomes to MDT.
  • Utilise highly developed interpersonal skills to facilitate communication, especially with barriers such as sensory or cognitive impairment or in hostile or highly emotive situations.
  • Establish and maintain robust communication networks with clients, carers, other health care workers and external agencies.
  • Promote recovery and recovery‑oriented practice through active collaboration and shared decision‑making.
Eligibility and Sponsorship

This role is not eligible for sponsorship unless you meet the criteria by temporary exemption from current changes to immigration rules put in force on 22/07/2025. Separate provisions apply to workers who have been sponsored and held continuously a Skilled Worker visa since prior to 04/04/2024.
