A leading mental health organization in Wythenshawe, UK, is seeking a professional to support service users in their recovery. This role involves working alongside multi-disciplinary teams to assess and implement individualized care plans. You will promote social inclusion and recovery-oriented practices, needing strong interpersonal skills to communicate effectively with service users and healthcare professionals. The post holder will work as part of the South Manchester Home Based Treatment Team, undertaking allocated duties within the team. They will work alongside mental health practitioners to support patients, families and carers in their own homes and to support patients who are on the community waiters and supported discharge pathway from the wards, ensuring the highest possible standard of care in accordance with policies and procedures.
Service users will have individualised care plans. The role focuses on the direct needs of service users and promotes social inclusion and active participation in an individualised recovery plan using agreed team values and skills.
This role is not eligible for sponsorship unless you meet the criteria by temporary exemption from current changes to immigration rules put in force on 22/07/2025. Separate provisions apply to workers who have been sponsored and held continuously a Skilled Worker visa since prior to 04/04/2024.
