Job summary

Abbeycare and Nursing LTD is seeking a Domiciliary Palliative Care Worker to support individuals in their own homes. You will provide personal care, assist with symptom management, and offer emotional support to families during life-limiting conditions.

You will work with district nurses, GP teams and hospice staff to deliver dignified, patient-centred care, ensuring comfort, safety and respect at all times in a community setting across Sheffield and surrounding areas.

Qualifications

• Experience in care or support roles, with palliative care experience desirable.
• Strong communication and emotional resilience in sensitive situations.
• NVQ Level 2 or 3 in Health and Social Care desirable.

Responsibilities

• Provide personal care and support with washing, dressing, continence care and comfort.
• Follow care plans, assist with safe medication prompts and report changes to clinicians.
• Offer reassurance, companionship and a calming presence to individuals and families.
• Ensure dignity, privacy and respect for personal wishes and cultural beliefs.
• Support end-of-life comfort and help with nutrition, hydration and environment safety.
• Document changes and communicate concerns to the Service Manager and clinical teams.
• Collaborate with district nurses, GPs and hospice teams for coordinated care.
