Job summary

Promni Health is seeking an Occupational Therapist to deliver home-based assessments and interventions for frailty in a forward-looking primary care network. The post involves travel between patient locations, with both full-time and part-time options on a fixed-term basis until 31 March 2027.

Driving to patients' homes is essential. You will work within a multidisciplinary team to support proactive frailty management, refine care planning, and improve patient independence and safety at home,

Qualifications

• Degree or MSc in Occupational Therapy.
• HCPC registration is required.
• RCOT membership is desirable.

Responsibilities

• Conduct home-based assessments and interventions for frailty in primary care.
• Coordinate with MDT for proactive frailty management and care planning.
• Support safe discharge and prevent hospital readmissions.
• Provide environmental risk assessments and home modifications for independence.
• Document care plans and ensure HCPC RCOT standards are met.
