Job summary

Harrogate and District NHS Foundation Trust is seeking a Community Healthcare Support Worker to join our Community Care Teams. The role runs across 7am–8pm with flexible shift patterns in the Harrogate and Rural area, requiring a valid UK driving licence and access to a vehicle.

We offer a full induction, ongoing professional development, in-house clinical supervision, and access to e-learning and career progression opportunities within the Trust. Closing date: 17 November 2026.

Qualifications

• NVQ Level 3 or equivalent or willingness to undertake
• Full UK Driving Licence and access to own vehicle
• Experience in health or social care environment is desirable
• Knowledge of Medicines Management and good IT skills

Responsibilities

• Assist in assessing people’s health and social care needs.
• Take and interpret physical measurements and samples as delegated by the team.
• Monitor changes in patients and inform the team when significant changes occur.
• Assist in planning, delivering and evaluating nursing care and rehabilitation programmes.
