Job summary

Community Health Action of Staten Island (CHASI) is seeking an Outreach Engagement Specialist to join the HIV Care Coordination team. You will perform outreach with community partners, medical facilities and locations frequented by the target population to ensure timely referrals into CHASI’s program and its medical partners.

Qualifications

• High School Diploma or equivalency.
• Ability to engage and serve diverse populations.
• Fluency in English; Spanish preferred.

Responsibilities

• Conduct street and facility-based outreach to engage HIV-positive individuals.
• Assist clients to schedule and attend medical appointments and document outcomes.
• Submit accurate data to CHASI’s Data Unit on outreach activities.
• Attend one-on-one check-ins with supervisor and department.
• Collaborate with staff to integrate cross-program activities.
• Align activities with CHASI’s mission and values.
