Job Summary

Home First are excited to advertise a Band 7 Physiotherapist Rehabilitation Navigator role. This fixed‑term contract is designed to meet winter pressures and to trial the navigator role; evaluation will shape future rehabilitation services.

Main Duties (Responsibilities)
  • Support the delivery of Home 1st Planned and Unplanned Care services, providing highly specialist holistic assessments and person‑centred discharge planning advice to patients requiring supported discharge from Milton Keynes University Hospital (MKUH).
  • Collaborate with MKUH integrated discharge and therapy teams to manage discharge plans for transfer to community‑based inpatient Seacole beds.
  • Liaise with other Home 1st teams, Milton Keynes Council social care teams and partner organisations to triage, allocate and expedite referrals.
  • Provide sign‑posting advice and improve the flow of patients into community services.
  • Appropriately manage clinical risk, maintain waiting lists and conduct risk assessments to safeguard patients.
  • Prescribe, order and review equipment needed for supported discharge and ensure safe, competent use of all appropriate equipment.
  • Attend multidisciplinary meetings at MKUH, the Virtual Ward and other relevant groups to represent community services and influence discharge decisions.
  • Maintain accurate electronic records and contribute to service audit to assess effectiveness of clinical pathways.
  • Supervise and support the development of Band 5 therapists on rotation.
