Job summary

The Royal Free London NHS Foundation Trust is seeking an Occupational Therapist to develop and deliver community-based care within the Enfield Proactive Care Service MDT, contributing to integrated neighbourhood team models and improving patient outcomes.

You will work with senior colleagues to provide specialist OT expertise, manage a caseload, coordinate assessments, and empower patients and carers to maximise independence in line with service standards.

Responsibilities

• Deliver a high standard of Occupational Therapy care to patients/clients and their families/carers in a community setting.
• Coordinate and deliver Occupational Therapy interventions within the service, managing a designated caseload.
• Ensure that all Occupational Therapy assessments are completed within agreed service timescales and quality standards.
• Support, educate and empower patients and their carers to manage complex rehabilitation and Occupational Therapy needs.
• Work alongside senior colleagues to provide specialist Occupational Therapy expertise and advice to the wider service and multidisciplinary teams.

This role offers an exciting opportunity to further develop the Enfield Proactive Care Service Multi-Disciplinary Team (MDT) model and contribute to the design and delivery of an exemplary Integrated Neighbourhood Team (INT)
