Qualifications

• Registered Nurse 1st level registration with NMC.
• First level degree, working towards/ equivalent experience.
• Extensive experience in cardiology.
• Evidence of continuing professional development.
• Ability to work autonomously and within multidisciplinary teams.
• Non-Medical Prescriber or working towards.
• Leadership training.
• Teaching qualification.
• Additional long-term condition management training.

Responsibilities

• Rotational band 7 post rotating between community and acute heart failure teams.
• Inpatient reviews, titration of medications, clinic management and home visits.
• Work collaboratively with cardiology, acute and primary care colleagues.
• Evidence-based care is disseminated throughout clinical areas.
• Support and training to staff as part of the role.
