Job summary

HCRG Care Group in Gravesend is seeking a Healthcare Assistant to join the Gravesham Central Community Team based at Gravesham Community Hospital. The role involves delivering care in patients' homes and local clinics, helping people stay independent in their own communities.

Main Responsibilities

In this community based role, you will support the delivery of nursing care to patients in their own homes and local clinics. This is a varied and active position where no two days are the same. You will play a vital role in helping patients manage their health, recover from illness and maintain independence. You will work closely with registered nurses and the wider multidisciplinary team, carrying out delegated care and developing your clinical skills. You will also have the opportunity to work independently, planning your visits and managing your time effectively across the local area. Key responsibilities include:

  • Supporting patients with delegated nursing care in their own homes
  • Carrying out clinical tasks such as dressings and glucose monitoring following training and competency sign off
  • Supporting holistic, evidence based care that meets each patients individual needs

This role requires travel across the local area, so a UK driving license and access to a car is essential.

Person Specification

Essential

  • NVQ Level 2 or 3 in Health and Social Care, or equivalent experience
  • A genuine interest in community based care and supporting patients in their own homes
  • Full UK driving license and access to a car
  • Good communication and interpersonal skills
  • Ability to work independently and manage your own workload
  • Basic IT skills including Microsoft Office

Desirable

  • Previous experience working in a community or domiciliary care setting
  • Experience supporting a wide range of patient needs in a non hospital environment
  • Confidence working autonomously in the community
