Job summary

HCRG Care Group in Gravesham seeks a Healthcare Assistant to join the Gravesham Central Community Team at Gravesham Community Hospital. You will deliver nursing care in patients' homes and local clinics, supporting independence and wellbeing in the community, with a role that suits active, mobile workers who enjoy traveling between visits.

You will work with registered nurses and a multidisciplinary team, plan your visits, maintain records, and develop clinical skills.

Qualifications

  • NVQ Level 2 or 3 in Health and Social Care or equivalent experience.
  • Interest in community-based care and supporting patients at home.
  • Full UK driving licence and access to a car.
  • Good communication and interpersonal skills.
  • Ability to work independently and manage workload.

Responsibilities

  • Support delegated nursing care in patients' homes.
  • Carry out clinical tasks such as dressings and glucose monitoring after training.
  • Support evidence-based care tailored to each patient’s needs.
  • Maintain accurate patient records using electronic systems.
  • Assist with administrative tasks to support the team.
  • Plan and organise daily visits across the community.

As a Healthcare Assistant, you will join our valued Gravesham Central Community Team based at Gravesham Community Hospital. This is a rewarding opportunity to deliver care where it matters most, in people’s homes and local clinics, helping individuals stay independent and supported within their own communities. This role is ideal for someone who enjoys working out in the community rather than being based in one location all day. You will build meaningful relationships with patients and play a key role in supporting their health and wellbeing in a more personal, one to one setting. You will be part of a supportive, close knit community nursing team that works together to deliver high quality, compassionate care across the Gravesend area.

You will be joining a welcoming and experienced community nursing team who are passionate about delivering personalized care closer to home. Main Responsibilities

In this community based role, you will support the delivery of nursing care to patients in their own homes and local clinics. This is a varied and active position where no two days are the same. You will play a vital role in helping patients manage their health, recover from illness and maintain independence. You will work closely with registered nurses and the wider multidisciplinary team, carrying out delegated care and developing your clinical skills. You will also have the opportunity to work independently, planning your visits and managing your time effectively across the local area. Key responsibilities include:

  • Supporting patients with delegated nursing care in their own homes
  • Carrying out clinical tasks such as dressings and glucose monitoring following training and competency sign off
  • Supporting holistic, evidence based care that meets each patient’s individual needs
  • Maintaining accurate patient records using electronic systems
  • Assisting with administrative tasks to support the team
  • Planning and organizing your own daily visits across the community

This role requires travel across the local area, so a UK driving license and access to a car is essential.

Person Specification
  • NVQ Level 2 or 3 in Health and Social Care, or equivalent experience
  • A genuine interest in community based care and supporting patients in their own homes
  • Full UK driving license and access to a car
  • Good communication and interpersonal skills
  • Ability to work independently and manage your own workload
  • Basic IT skills including Microsoft Office
  • Previous experience working in a community or domiciliary care setting
  • Experience supporting a wide range of patient needs in a non hospital environment
  • Confidence working autonomously in the community
