Job description

Kent Community Health NHS Foundation Trust

Healthcare Assistant

The closing date is 12 November 2026

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare or working on the Staff Bank within these organisations. Nursing and AHP vacancies are also open to Student Nurses and Newly Qualified Nurses/AHPs.

Are you passionate about helping people remain independent in their own homes and avoiding unnecessary hospital admissions? If so, this could be the perfect opportunity for you.

As part of our Urgent Care Services team, you will play a vital role in providing short-term health, therapy and care support to people across the community. Working alongside Nurses, Therapists and other healthcare professionals, you will help deliver personalised care to patients who are acutely unwell, recovering from illness or injury, or transitioning home following a hospital stay.

This rewarding role offers a diverse workload where you will support individuals to achieve their goals, regain confidence and maximise their independence. From monitoring health needs and supporting rehabilitation programmes to assisting with activities of daily living, you will make a meaningful difference to the lives of patients and their families every day. If you enjoy building relationships, problem solving and seeing the positive impact of your care, you'll thrive in this dynamic and supportive team.

Main duties of the job

This is an exciting opportunity to develop and utilise a wide range of clinical and rehabilitation skills as part of a multi-professional team. Working under the guidance of Nurses and Therapists, you will deliver elements of prescribed care packages while contributing to the ongoing assessment, implementation and evaluation of patient care.

Your role will include undertaking delegated clinical activities such as baseline observations, glucose monitoring, urinalysis, wound care, equipment provision and falls prevention interventions, following competency-based training. You will also support patients to increase their independence with everyday activities and, where appropriate, assist with delivering individual and group-based therapeutic interventions.

We're looking for someone who is compassionate, motivated and committed to providing high-quality patient-centred care. In return, you'll gain valuable experience across a broad range of clinical settings, work alongside experienced professionals and be part of a service that helps people stay safely at home, recover well and achieve the best possible outcomes.
