Role Overview

Under the supervision and support from the team you will be required to visit patients in their own homes and provide clinical care. We are seeking a highly motivated, enthusiastic individual with good communication skills and a passion for caring for patients in their own homes. There is an expectation that the successful candidate will undertake our in-house development program for extended skills which will involve insulin administration, injections and wound care. Responsibilities & Expectations
  • Promotes a 'can do' attitude and determination to develop and sustain a dynamic and responsive administrative and operational service within a community setting which delivers best practice.
  • Able to implement interventions, liaising with colleagues and partners to ensure an integrated approach to the needs of the service user.
  • To organise and deliver skilled clinical support work without direct supervision.
  • To manage clinical risk within delegated patient caseload, referring to the supervising clinician for support and supervision.
