We are looking for an Integrated Care Worker to join our East and South City Nursing Team. This exciting opportunity will allow you to onboard as a new member to the Cambridge East and South City Nursing team, where you will be fully supported to gain new skills and experience needed to deliver high quality care to patients in their own homes or residential settings in and around the area.
We provide a seven‑day a week service, working closely with other community services and Primary Care Networks to deliver high standard holistic care to the population of Cambridgeshire.
You will have the opportunity to develop yourself with extensive learning and development opportunities.
Please note for this role you will be required to travel independently around the county, meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. Public transport is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.
As an Integrated Care worker you will work closely with the nursing team to follow a plan of care and deliver high quality, best‑practice care to adults with a range of complex health needs within their own homes.
