Job summary

A community health organization is seeking a Health Link Worker for their Smokefree Lancashire service in North Lancashire. Smokefree Lancashire has been commissioned to deliver the stop smoking service across 12 districts of Lancashire. Our goal is to make smoking cessation a priority for everyone. We provide advice and information to people who want to quit smoking and develop personalised plans during one-to-one support sessions. We also offer groupwork sessions, pharmacotherapy and deliver training to our partner agencies.

We have an exciting opportunity for a Health Link Worker to join our Smokefree Lancashire service. The new post holder will join our dedicated team, providing advice and information to people who want to quit smoking within hospital settings.

Location North Lancashire

Hours Full Time 37.5 hours per week

Full Time Salary Range £25,077.00 - £26,502.53 dependent on experience (Based on full time hours, pro rata for part time)

Key Responsibilities
  • To promote the benefits of stopping smoking and the stop smoking services through a variety of mechanisms and at a range of venues and events.
  • To work as part of the specialist team providing brief advice and structured programmes to a wide population of individuals across the county.
  • Work as part of the NRT, Champix and E-cigarette protocol.
  • Set up effective communication and referral pathways with hospital-based specialist teams. Act as a point of contact for referrals.
  • Develop specific links with identified specialist teams and provide in reach to hospital wards including Respiratory Wards, Cardiology Wards, Maternity and A&E.
  • Work with community teams to ensure assertive and effective follow up of both repeat presentations and discharges from the hospital.
  • Provide training in relation to the harm of smoking and smokefree places to acute, and specialist colleagues.
About you
  • Experience of working within a Health and/or social care environment.
  • Understanding and knowledgeable of the harmful effects associated with smoking.
  • In-depth knowledge and understanding of the treatments and interventions available to smokers.
  • Possess excellent communication skills and approachability for hospital staff and patients alike.
  • Able to work autonomously when needed, representing CGL with confidence and integrity.
  • To promote the benefits of stopping smoking and the stop smoking services.
  • Be able to work as part of a team.
  • Willingly work flexibly across operational sites as required.
