Job Summary

A Health Care Support Worker works alongside a team with the Care Home Team, 18 care homes and wider PCN teams. We have a permanent vacancy for a Health Care Support Worker to join our large, supportive, busy and friendly team. The successful candidate will play an important role in complementing our current Care Home Team and other members of the PCN multi‑disciplinary team.

Job Description

North Lincolnshire South Primary Care Network (PCN) is looking to expand their Care Home Multidisciplinary Team. We are recruiting an experienced Health Care Support Worker for our care home team in support of patient care and work in line with the Network Contract Directed Enhanced Service (DES) Specification. The post holder will be delivering care and developing the service according to the principles of the Enhanced Health in Care Homes framework and working closely with practices to deliver outstanding patient care.

The post holder will work closely with 18 care homes and the 6 practices, including Ancora Medical Practice, Ashby Turn Primary Care Centre, Cedar Medical Practice, Cambridge Avenue Medical Centre, Kirton & Scotter Surgery and West Common Lane within the PCN and other local organisations.

Key Responsibilities
  • Carry out clinical assessments on residents.
  • Perform blood pressure checks and act on the results.
  • Carry out venepuncture or undertake training as required.
  • Follow protocols, work within scope of competency and provide feedback to registered nurse.
  • Use own initiative and work without close supervision.
  • Listen effectively and demonstrate a sympathetic/sensitive approach; speak effectively to the wider MDT, patients and families.
  • Record vitals and assessments: oxygen saturations, pulse rate and rhythm, respiratory rate, temperature, height and weight/MUAC, waist circumference and BMI, visual acuity, limb measurements and assessment, blood glucose monitoring, history documentation, peak flow readings.
  • Use and complete clinical tools to aid registered nurse decision‑making.
  • Support delivery of enhanced services and other service requirements on behalf of the PCN.
  • Attend and participate in multidisciplinary teams (MDT) within PCNs when required.
  • Undertake all mandatory training and induction programmes.
  • Contribute to and embrace the spectrum of clinical governance.
  • Develop yourself and the role through participation in training and service redesign activities.
  • Attend a formal appraisal with manager at least every 12 months; review progress regularly.
  • Contribute to public health campaigns (e.g. flu/COVID clinics) through advice or direct care.
  • Maintain a clean, tidy, effective working area at all times.
  • Support delivery of QOF, incentive schemes and other quality or cost‑effectiveness initiatives.
  • Undertake tasks consistent with level of post and scope of the role, ensuring timely and effective delivery.
  • Duties may vary from time to time without changing the general character of the post or level of responsibility.
  • Work within a team and use own initiative to work independently.
  • Attend weekly debriefs and monthly team meetings on Wednesdays.
Person Specification
Qualifications (Essential)
  • Hold Education Level 3 NVQ with evidence.
  • Experience of taking clinical observations and interpreting the readings.
  • Clean driving licence and own transport.
  • Ability to follow protocols and work within scope of competency and provide feedback to ANP/Clinical Director.
  • Communicate in a sensitive and reassuring manner, adhering to confidentiality at all times.

Experience (Essential)
  • Previous experience of working as a healthcare assistant in a hospital, registered care, community setting or GP practice.
