Overview

We are delighted to offer an exciting opportunity as a Healthcare assistant for the Hospital at Home team. This is a combination of community and acute working.

Responsibilities
  • Work within the Hospital at Home team to contribute to ongoing assessments of patients.
  • Provide and monitor patient care in line with service pathways.
  • Monitor condition and health needs of patients on a continual basis.
  • Establish effective communication with patients/carers to enable therapeutic relationships.
  • Visit patients at home to review progress against established pathway.
  • Escalate concerns to clinical team where required.
  • Provide home visits to obtain blood samples, capillary blood gases, ECG, delivery equipment and other clinical tasks.
  • Assist in remote monitoring of patients.
  • Work alongside the MDT.
  • Support patients to improve and maintain mental, physical, behavioural health and well-being.
  • Promote comfort and well-being by ensuring patients’ personal and social needs are met.
  • Provide evidence-based care for service users under supervision of registered nurses.
Qualifications

Experience in community and acute settings. Ability to perform venepuncture, capillary blood gases, ECG. Good communication skills. Compassionate, safe and effective care. Ability to work with MDT. Support for remote monitoring.
