Job Overview

We have a unique and exciting opportunity for an enthusiastic individual who is interested in working within the Discharge Lounge, Wexham Park Hospital.

The Discharge Lounge is a 7 day week Service operating between 7-7. Our aim is to provide a comfortable, safe and efficient environment for our patients transferring from hospital to home.

The Lounge provides support to ward areas, patient flow and to the patients within the lounge until they are discharged safely home.

We are looking for a motivated and self‑driven individual whose priority is to deliver high quality patient care and service throughout the Discharge Process.

The successful post holder will be friendly, enthusiastic and adaptable, and be able to demonstrate flexibility to meet these service demands, as well as having the creativity to develop the service. This is a highly physical role and involves a lot of walking.

Main duties of this role

As a Health Care Assistant, you will work under direct or indirect supervision of a Registered Nurse or Nursing Associate. You will primarily assist the nursing team with patient care and facilitate the smooth running of the department.

In this front‑facing clinical role you will support your nursing colleagues, act in the best interest of patients and provide excellent customer care to patients, relatives, carers and colleagues.

Specific Main Duties Within Our Team

• Collect patients from ward areas and transport them into the Lounge.
• Provide basic nursing care while in the lounge.

Detailed Job Description And Main Responsibilities

These are some of the clinical roles you may be expected to deliver; please refer to the full JD for a more expanded list.

• Care for enhanced care observation patients (1:1) or bay‑based observation as required.
• Support and assist professionals in the implementation of the care plan for individual patients.
• Assist patients to eat and drink as required, including menu selection and awareness of special diets and cultural requirements.
• Enable and assist patients to maintain personal hygiene.
• Contribute to the safe movement of patients, mobilise and move patients using equipment to ensure patient comfort after a mobility assessment.
