Health Care Assistant (Domiciliary Care/Home Care/Community Based Support)

Position: Full-Time/Part-Time

Location: South Gloucestershire & North Somerset

Role Overview

We are seeking compassionate and dedicated Care Assistants to adults in the community. You will be responsible for delivering high-quality care that promotes independence, dignity, and well-being of people by providing responsive care in their homes. This role involves assisting clients with daily living activities, personal care, medication support, and companionship.

Key Responsibilities
  • Provide personal care, including washing, dressing, and toileting, while maintaining dignity and respect.
  • Assist with safe repositioning and use of mobility equipment (profiling beds, slide sheets).
  • Promote and protect the dignity, privacy, and preferences of those in care. Assist with meal preparation, feeding, and promoting healthy nutrition.
  • Support with medication administration in line with care plans. Help with household tasks such as cleaning, laundry, and shopping.
  • Offer companionship, dignity-focused support and emotional support, encouraging social interaction.
  • Apply skin care protocols and maintain skin integrity. Work as a team for scheduled visits.
  • Support individuals with mobility needs, using appropriate equipment as required.
  • Monitor and report changes in clients' conditions and comfort levels to relevant healthcare professionals. Ensure compliance with care plans, safeguarding policies, and health & safety regulations.
  • Provide waking night support (as needed), staying vigilant throughout.
  • Support families sensitively through anticipatory grief and post-death processes (e.g., notifying DN or GP).
  • Accurately record observations, actions and concerns via digital or paper logs.
Requirements
  • Experience in domiciliary care, community care, supported living, mental health, learning disabilities, or behaviours that challenge is desirable.
  • Knowledge of anticipatory medicines, symptom observation. A caring, patient, and empathetic nature. Excellent communication and interpersonal skills. Ability to work independently and as part of a team.
  • A flexible approach to work, including availability for evenings and weekends. Enhanced DBS. A valid UK driving licence and access to a vehicle is critical. Manual handling training and experience with equipment.
Desirable
  • NVQ Level 2 or 3 in Health and Social Care (or working towards).
  • Experience supporting individuals with stomas, catheters or NG feeding.
  • Experience of working with people with fluctuating capacity and on DoLS orders.
