Health and Wellbeing Coach

We are looking for a motivated and compassionate Health and Wellbeing Coach to join our Primary Care Network (PCN) team in North West Surrey. You will work with patients to support them in improving their health and wellbeing through personalised, non‑judgemental coaching approaches.
You will manage a caseload of patients, helping them to identify goals, build confidence, and develop the skills needed to better manage their health and live independently. You will work closely with GPs, the multidisciplinary team and social prescribing services to ensure coordinated, holistic care.
Work may be full time or part time. The role is hybrid, with expectation to hold face‑to‑face clinics at one of the GP sites from the PCNs involved in this pilot. Some clinics will be telephone clinics and done from home.
Main duties of the job
- Deliver one‑to‑one health coaching using a person‑centred approach.
- Support patients to set and achieve personalised health and wellbeing goals.
- Identify patients who would benefit from health coaching via MDT and IT systems.
- Work in partnership with social prescribing services and community organisations.
- Encourage self‑management, behaviour change and shared decision‑making.
- Maintain accurate records and manage risk appropriately.
Job responsibilities
- Manage and prioritise a caseload in accordance with the health and wellbeing needs of the local PCN population using a non‑judgemental, communication‑focused approach.
- Refer patients back to other health professionals within the PCN when appropriate.
- Utilise existing IT and MDT channels to screen patients, aiming to identify those who would benefit most from coaching.
- Provide personalised support to individuals, families, and carers, empowering them to manage their own health and wellbeing and live independently through coaching and motivational support.
- Facilitate self‑management education and peer‑support interventions.
- Support patients to set and attain meaningful goals, encouraging proactive prevention of further illness.
- Partner with the social prescribing service to connect patients to community‑based activities that increase self‑management and healthy behaviours.
- Help patients develop knowledge, skills and confidence to engage with healthcare and participate in shared decision‑making conversations.
- Explore and support access to a personal health budget where appropriate.
- Develop collaborative relationships with health, social care and community providers to holistically support patients and reduce local health inequalities.
- Provide education and expertise to PCN staff, raising awareness of shared decision‑making and decision‑support tools.
- Co‑operate with a GP supervisor and a health coaching mentor for regular supervision.

Person Specification
Knowledge and Understanding
- Knowledge of the wider determinants of health, including social, economic and environmental factors.
- Understanding of how lifestyle behaviours impact health and long‑term conditions.
- Knowledge of confidentiality principles and how they apply to handling patient information.
- Understanding of personalisation, health coaching and the skills required to support self‑care.
Experience
- At least two years experience delivering face‑to‑face support in a related environment.
- Experience adopting a person‑centred approach and empowering people to make informed choices.
- Formal individual and group coaching supervision from a suitably qualified or experienced individual.
- Experience partnering with Social Prescribing Link Workers or services to support people not yet ready for full social prescribing.
- Experience supporting people with long‑term health conditions to make lifestyle changes.
Qualifications
- Completed health coaching training covering topics outlined in the NHS England and NHS Improvement Implementation and Quality Summary Guide, delivered by a training organisation listed by the Personalised Care Institute.
- Motivational interviewing, behaviour change, or health coaching qualification, or willingness to undertake training.

- Strong interpersonal, listening and rapport‑building skills.

- Capacity to work unsupervised to meet targets and deadlines.

- Demonstrated personal accountability, emotional resilience and ability to work under pressure.
