A healthcare provider in Greater London is seeking an experienced HCA/Care Coordinator to work 15-20 hours per week supporting the nursing team in delivering patient care. The role involves assisting with clinical support, managing patient care plans, coordinating access to services, and promoting health education. We are looking for an experienced HCA/Care Coordinator to work15-20 hrs per week over 2-3 days to support our friendly practice nursing team inthe delivery of our nursing services. The successful candidate will be workingas part of the practice multidisciplinary team, delivering care within theirscope of practice to our patient population. The successful candidate willdeliver and assist clinical staff in the provision of treatment, preventativecare, health promotion and patient education.
Working under the direct supervision of the GPs andexisting HCA and strictly in accordance with specific practice guidelines andprotocols, the healthcare assistant will assist the practice clinical team inthe provision and delivery of prescribed programmes of patient care. Directingpatients to locally available support and services whilst using clinicalsystems on a daily basis, with confidence to learn advanced techniques and keepcare records up to date. Supporting the coordination of all keyactivity, including access to services, advice and information, as well asensuring that health and care planning is timely, efficient, and patient centered.
